Data Request Form
The standing package we ask the City of Indianapolis to provide for any school and any period, with the reason each document is needed and what is reported when it is not supplied.
What we do with the package
Nsites reproduces each Core Question 2 sub-indicator independently from the school's own source documents. Every figure we publish is traced to a named file and a page or sheet-and-cell reference, so any number in the finished report can be clicked back to the exact line it came from.
What we never do
Published OEI reports, Salesforce/CAP extracts and prior-year evaluations are never used as calculation inputs. They are held aside as comparison targets so that agreement between our result and OEI's result is meaningful rather than circular.
Why format matters
Text-based PDFs and native Excel workbooks can be parsed line by line with a page or cell reference retained. Scanned images and PDF printouts of spreadsheets cannot be traced automatically and require manual transcription, which slows the review and weakens the audit trail.
What happens when something is missing
We do not estimate, infer or copy a published figure to fill a gap. Any sub-indicator without complete source evidence is reported as Unable to Validate, and the specific missing document is named in the report and in Exceptions - Review Needed.
